Or were the lateral incisors too small?













C did not like her smile, and felt that her two front teeth (central incisors) looked too big. When we analysed her smile however, we felt that the problem was that the two teeth beside the front teeth (lateral incisors) were too small, with old and stained white fillings. SO, rather than reduce the two central incisors – we added to the laterals. This is a more conservative treatment, and creates the impression of a more even and harmonious smile.

Treatment involved an initial course of whitening, followed by a try-in of porcelain veneers before finishing with cosmetic bonding. Interestingly, this was a case where cosmetic bonding proved to give us a better cosmetic outcome that porcelain veneers.

Treatment : J was unhappy with the appearance of his teeth, but didn’t want to use porcelain crowns to rebuild them. We discussed all the treatment options and decided that he would be suitable for tooth whitening and cosmetic bonding. We whitened the teeth first using slow release combined with internal bleaching, and then rebuilt the teeth using cosmetic bonding.
This stunning result took just a matter of weeks, and has totally transformed the J’s appearance.

Patient : Sinead, aged late 20’s early 30’s presented with very worn front teeth near the gum level. This wear was partly due to the crowding of her teeth so we treated her orthodontics first of all. Now that her teeth were straight, we needed to improve their appearance.

Treatment : This patient did not like her discoloured front teeth, and the failing composites which had been placed some years beforehand. We discussed all the treatment options including porcelain crowns, veneers and cosmetic bonding. She decided to opt for cosmetic bonding because :
a) it is possible to do in a single appointment,
b) reduced cost compared to other treatments,
c) although not quite as cosmetic as porcelain veneers, it is still possible to get a very good aesthetic result.
Cosmetic bonding was performed on the two front teeth in a single appointment. These composites are a big improvement on the originals, but it is likely that they will need to be replaced in about 7-8 years’ time

Treatment : This patient had short and rotated lateral incisors (the teeth either side of the two front teeth). She did not like the way that this made her smile look uneven with big gaps between the teeth. We discussed all the treatment options including porcelain veneers, short term orthodontics, and cosmetic bonding. She chose cosmetic bonding because of its quick treatment time (single appointment) and reduced cost. Cometic composites were placed on the two lateral incisors and canine teeth to reduce the gaps and to make the teeth more level. Matching the shade of the composite exactly to that of the surrounding teeth can be one of the biggest challenges in dentistry, but if done correctly, the results can be dramatic.

This 25 year old patient broke his front tooth playing football; he was naturally upset and at first glance, it looks pretty bad. In the past, we would have needed a crown to repair it

In the past, we would have needed a crown to repair it, but with modern cosmetic bonding materials, we can add directly to the broken tooth and craft the material to the exact shape and colour of the original. This was done in a single appointment, on the same day as the injury occurred.

In dentistry, there are a number of different solutions to every problem; in fact deciding which treatment is best suited for each individual patient can be as challenging as actually carrying out the treatment.

This patient did not like the appearance of her smile, in fact she had hated it for most of her life, and hardly ever smiled. When I aksed her why she had never done something about it before, she told me that she was worried:�

c) she absolutely did not want braces
d) she wanted them to look better – they didn’t have to look “perfect”

When I recently received an email from Miss Universe Ireland 2010 Rozanna Purcell, asking me if I could improve her smile, I was delighted to be able to help her.

Now the difference between a perfect smile and a not-so-great one is about 1/10 of a millimeter, so there’s no room for error, particularly if a patient’s career depends on looking beautiful. Cosmetic dentistry can be at turns rewarding and challenging, and the concentration required can be pretty draining; still I’m not complaining, there are tougher jobs.

What we did:
Roz didn’t really like the appearance of her upper front teeth, she felt that they looked too short and grey at the bottom. We agreed that by removing the failed composites and replacing them with a whiter, less translucent cosmetic bonding, we could dramatically improve her smile. Interestingly, the teeth appear longer even though they aren’t by much; it’s just that the better shape and colour makes them appear longer.

What I like about composite bonding is that it is a conservative treatment; we removed none of Roz’s enamel from her natural teeth, and the treatment was all carried out in a single appointment.



So now that the cosmetic bonding is completed on the top teeth, Dr. Helen Walsh is taking over Roz’s care to complete the orthodontic straightening of her lower teeth (update and pics to come in a few months!).

I want to thank Roz for being a great patient, my somewhat ignorant expectation of tantrums and tiaras proved to be miles off the mark!
